Basic Band Forms

Field Trip Permits
Below is the list of forms to be turned in before the 1st football game. Before going on any trip by bus the Band needs permission slips filled out on all students. Some are CCSD forms and some are Basic High forms. All are required to be signed and turned in before any student takes a trip. Dates of trips and places are to be left blank so we can use these forms for all trips and not need a new set for each trip. These forms are included in the Band Handbook handed out at the beginning of Band Camp. |
Absence FormAll absences require a one week notice and approval by the band director. The form for absences is available in the band office or click here to print one. Please do not expect the band director to remember any conversation about an absence. Use the form to get your absence approved before you are listed as a no show and lose points. If you are ill use the form within 2 days of returning to school to get your absence excused.June 28, 2008 |
Smart MusicThe Clark County Secondary Fine Arts Department has made available a subscription to Smart Music. This program is one of the premiere music programs to practice music. To learn more about the program you can go to Smart Music. Your cost is $25 for a year subscription, you will also need to purchase a microphone for your computer for $19.95. To order Smart Music on line use the code Basicband08 the usual cost for the subscription is $100 a year - you save $75 with a subscription from the school. Computers will be available in the band room for students to practice with Smart Music at school. We will be glad to give you a demonstration of how the program works and why it will be beneficial to your child. Mr. McArthur will be using this program in class to test students - students will get first hand experience with Smart Music. Concert Band Concert Attire
The band program requires a FORMAL Concert Dress for the girls,and Tuxedo shirts, black bow tie and cummerbund for the Concert Band boys. Form will be available soon to order these. Black dress pants must be purchased separately. Dress pants are required for the formal dress for the Concert Band boys, not Dickies, jeans, or carpenter pants. If you are unsure if your pants are appropriate please have them checked with the band office. Dress shoes are also required.

Wind Ensemble Concert AttireThe band program requires a FORMAL Concert Dress for the girls; Tuxedo Jacket and pants for the Wind Ensemble Boys. We will be ordering dresses for the girls, tuxedo jackets and pants, and tux shirts, cummerbunds and bow tie for the boys (theirs to keep) in October. The dress/ tux set order form is due in the band office by October 30, 2007. We are ordering a different dress than in the past for the girls so the cost will be updated when more information is available. The dress usually runs $80 for the girls. A form will be available soon and includes a sizing chart for you to decide the appropriate size for your daughter. The band will pay for shipping. The order form for the tuxedo jacket and pants for the Wind Ensemble boys will be available soon. Cost for a full tuxedo set usually runs $100. The form will also includes prices for just the individual items. This will be worn at least 5 times throughout the concert season and is more cost effective than renting. Boys may also go to any tux shop in town and purchase a tux locally. The tux shirt we are using is the wing tip shirt. The tux, cummerbund and bow tie must be black.August 29, 2007 |
